Dial *170# and select the below options:

1. Select option 5 for Financial Services
2. Select option 5 for Insurance
3. Select option 2 for Allianz4Life
4. Select option 3 for Claim

Under My Claims, you will see the below. 

1. Death claim

2. Hospitalization claim
