  1. Enter Phone number
  2. Enter OTP
  3. Select MoMo REACTIVATION/CLAIM FORM
  4. Generate Forms
  5. Enter bio data details
  6. Proceed to submit your answers
  7. An SMS will be sent with Ticket Number

NB: You can follow up with Ticket Number via any of our digital channels if request is not completed after 24 hours
